Social Prescribing Link Worker

County Kildare Leader Partnership in partnership with Sláintecare Health Communities are delighted to offer this exciting opportunity for a dynamic individual with a strong track record in making a difference in people’s lives to work with individuals who are struggling with isolation, loneliness and feeling disconnected.

The Social Prescribing Link Worker will be responsible for working with individuals on a one- to-one basis to improve health and wellbeing as well as working in partnership with health professionals and the community and voluntary sector. Social prescribing involves taking referrals from individuals seeking support as well as from healthcare professionals and supporting them to connect and participate with a local community activity or service they have an interest in engaging with.

If you have a track record of working with people and are energised by the potential to improve people’s quality of life by working in partnership with local agencies to raise awareness of social prescribing and how partnership working can improve health outcomes and enable a holistic approach to care health and wellbeing, then this job may be for you.

To be a successful candidate you will need to hold a third level qualification in the area of Educated to a third level qualification in Social, Community, Health, or related field, have a minimum of 3 years’ experience in community development or health / care setting. have excellent communication and interpersonal skills as well as a commitment to delivering a quality service.

You will also need to have a full driving license and own transport. An initial one-year fixed term contract will be offered to the successful candidate. Competitive salary offered.
