The RC aims to help identify a prisoner’s resettlement needs and develops a release and post-release plan that offers the best chance of success for that person on release from prison.

The aim of the Community Support Scheme is the structured release of people from prisons with each individual being supported and case managed to include appropriate referrals to community-based programmes.

The role is specifically associated with the Sláintecare Integration Innovation Fund (SIIF) and will involve the provision of case management and monitoring of a portfolio of projects funded across various rounds of SIIF funding.
