The Role
We are seeking an experienced Project Worker to support the Homeless Primary Care Team (HPCT) and Inclusion Health Hub (IHH) in delivering essential healthcare services to homeless and vulnerable populations.
Key Responsibilities include client care coordination, advocacy, administrative support, and outreach to ensure dignified and professional service delivery. The role involves collaboration with multidisciplinary teams and developing referral pathways.
Requirements include a third-level qualification in social care, psychology, sociology, or related work with vulnerable groups, with at least 2 years' relevant experience; expertise in the homeless sector and working with marginalised groups; strong multitasking, decision-making, scheduling, and problem-solving abilities. Excellent communication and teamwork skills. Fluent English.
