The Role:

The Community Connector will

  • Be responsible for creating opportunities for people living with an intellectual disability to build and maintain social roles and relationships
  • Work directly with each person, their wider circle of support and other community partners/groups to identify and increase support opportunities in the community
  • Will work collaboratively with the individual, their keyworker and the individual’s circle of support as appropriate
  • Identify supports that assist individuals to make their own choices

Skills and experience required for the role:

  • A relevant qualification, minimum level 7 in health, social care, community, or project development
  • Three years of experience working in community, health, or social care.
  • Experience in providing personalised support, focusing on social roles and relationships
  • A natural networker who routinely contributes within their own community and is skilled in connecting people
  • Understanding community development and supporting active citizenship
  • Ability to work independently and with others
  • Experience in teamwork and influencing others to achieve results.
  • Experience in supporting people to be self-advocates and drive change.
  • Proven ability to meet targets and adhere best practice.
  • Proficient in Microsoft Office, report writing, HSE Safeguarding policies, and strong communication, ICT, and organizational skills

Preferred Skills, Knowledge & Experience:

  • Demonstrates a commitment and experience in building community connection and supporting self-directed services
  • Experience in helping people find jobs and develop career plans
  • Experience and participation in community groups and initiatives
  • Project management skills, including planning and analysis
  • Knowledge or experience in using Assistive Technology

The Community Connector Initiative 2824

Contribution, connection and belonging are things that make all our lives meaningful. For any people with a disability, this needs to be intentionally built. Community Connectors will lead on creating the pathway and support when required to enable each person to be active in their community. This will support SJOG with continued learning and strategy development on how to support communities to welcome, encourage and strengthen the participation of people with a disability to promote active citizenship, community inclusion while using a person centered approach.

Community connectors will work on a one-to-one basis with each person for a set period of time and the level of support will vary depending on people’s goals and needs.

The expectation is that the person becomes independent and/or will be supported by their circle of support.

St John of God Intellectual Disability Services; Liffey Services

Supports over 85 children and adults with intellectual disabilities in the Liffey Services region. Providing a range of Respite, Residential, Day Services and Early Services and two schools. The region expands across Dublin South-West and North and into Kildare.
