We are currently recruiting for the role of a Social Prescribing Link Worker. The contract will run for a 12 month period and is renewable subject to funding. Salary is commensurate with experience and will be based on the Project workers scale with a starting point of circa €40,000.

The Role

To support the implementation and development of the DSCP Social Prescribing Project. This is a full-time position (a 28 hour week may be available subject to negotiation) delivering a person-centred service to people experiencing health difficulties. The operational area to be covered is Bluebell, Inchicore, Crumlin, Drimnagh and Walkinstown. The role entails working on a one-to-one basis with people in targeted communities to link in with a variety of local, community-based activities and supports in their community whilst contributing to improving the individuals’ health and wellbeing, their self-management skills as well as their social connectedness within their local community

Key Duties and Responsibilities

Key responsibilities include but are not limited to the following:

  • To implement referrals, provide holistic assessments and co-design social prescriptions.
  • To keep accurate records and make regular reports.
  • To develop new links with local communities, specific GPs and primary care centres in targeted areas to widen the reach of the DSCP Social Prescribing Programme.
  • To implement referrals from participating community groups, GPs and other primary care health professionals, providing holistic assessments, co-designing a social prescription to improve health and well-being outcomes for individuals with a longer-term outcome of improving self-reported wellbeing and reducing the number of clinical /medical interventions required.
  • To build and maintain a comprehensive map of local resources which are available to service users
  • To manage referrals and cases on the relevant systems as well as using validated evaluation tools to measure health and wellbeing outcomes.
  • To keep accurate records and adhere to confidentiality, information sharing protocols and provide monitoring information as required.
  • To adhere to Data Protection policies and practices to ensure compliance with GDPR.
  • Any other duties relevant to the role at may be assigned from time to time. 

Person Requirement:

The ideal candidate will have:

  • A relevant third level or professional qualification, ideally in a health, social care-related or community development area.
  • Minimum of 2+ years’ experience of working within the community and / or health / care setting
  • Good understanding of multi-stakeholder working and the local and community development sector
  • A clean driver’s licence and the use of a car or other transport to enable travel around the areas covered by the Social Prescribing Programme
  • The Garda Vetting procedure will apply to this role

  • Maintain the highest level of confidentiality, discretion and professionalism at all times
  • Demonstrate capability to conduct one’s self in a calm and professional demeanour when dealing with the public and/or with difficult situations
  • A proven understanding of safeguarding for children and vulnerable adults and ability to implement relevant policies and procedures
  • Have excellent time management and record keeping skills
  • Be proficient in Microsoft Excel, Word, PowerPoint
  • Flexibility in adjusting hours to accommodate the needs of the role
  • At all times, adhere to the values of DSCP 

Reporting Relationships

The Social Prescribing Link Worker will report to the Workability Coordinator.
