Your Role as an Addiction Treatment and Aftercare Project Worker

Cork Simon Community is currently seeking motivated and compassionate individuals to join our team as an Addiction Treatment and Aftercare Project Worker. In this vital role, you will coordinate access to addiction treatment services, provide aftercare housing and support, and promote long-term recovery pathways for individuals experiencing homelessness.

This is an incredibly rewarding role that allows for continued learning and professional development, guided by Cork Simon’s values and best practice care models such as Trauma-Informed Practice and Harm Reduction.

Some Key Responsibilities:

  • Facilitation and Coordination: Rapidly assess and facilitate access to addiction treatment services for clients, ensuring seamless transitions and continuity of care.
  • One-to-One Support and Case Management: Provide personalized support and case management for residents in pre-treatment and aftercare phases, including assistance with housing, training, and recovery programs.
  • Collaboration and Networking: Build and maintain strong links with addiction treatment providers, community organizations, and homeless services to ensure coordinated care.
  • Promotion and Advocacy: Promote the project to residents, service users, and stakeholders, encouraging participation in treatment and recovery programs.
  • Administrative Duties: Maintain detailed case notes, care plans, and high-quality reports while adhering to data protection and reporting requirements.
  • Health and Safety Compliance: Ensure compliance with health and safety policies, conduct property checks, and participate in relevant training.
  • Team Collaboration: Work collaboratively with various Cork Simon Community teams and external partners to support project objectives and provide coverage when needed.
  • Professional Development and Performance Monitoring: Engage in supervision, professional development, and performance monitoring through key indicators such as effective communication, teamwork, and positive client outcomes.

Ideal Candidate Profile:

  • Experience: At least two years of experience working with marginalized groups, especially people experiencing homelessness or addiction, and case management experience.
  • Knowledge: Understanding of recovery orientation, addiction treatment models, and harm reduction strategies.
  • Skills: Strong case management, care planning, advocacy, record-keeping, and report-writing skills.
  • Qualifications: Relevant third-level qualification in social care, drug and alcohol training, first aid certification,
  • Full clean driving license, and access to a car.
